Saving, opening and using the forms

Please refer to these instructions for saving and opening the forms. If you are not familiar with how our forms work, you may want to read our user guide. If you continue to have difficulty using the forms after reading the guide, please contact: AGCSBSmartforms@gov.bc.ca.

Adobe Reader required

You will require the latest version of Adobe Reader in order to use the forms. If you need to update your Reader software, please ensure you remove the old version of Reader before installing the new one. You may also want to be sure that your computer is not starting Adobe Acrobat instead of Reader when opening the forms.

Computer settings

Check your computer settings to make sure the CSB Smart Forms website is added as a trusted site by following the applicable instructions below:

 

Google Chrome and Firefox users

If you are using Google Chrome or Firefox as your browser you will need to do a onetime configuration on your browser before using the forms. Follow these steps to configure your browser.

 

Apple and Mac users

Currently, the forms are not compatible with Apple products, including Mac computers, iPads, etc. If you are using a Mac computer, you can download the desired document and follow the instructions to configure your computer.

 

Microsoft Edge users

If you are using Microsoft Edge as your browser you will need to do a onetime configuration for the form to open within Adobe.  Follow these steps to configure your browser.

You can also use the forms by saving them to your hard drive. To do this, right click on the form you wish to use and save it to your computer.

 

Criminal forms

This is a complete list of all forms. By default they are sorted alphabetically, but this can be changed by clicking on the table headings below. For example, click on "Form number" to sort the forms numerically. You can also search by form name or number.

A Victim Impact Statement is a written description of how a crime has affected the victim. A Victim Impact Statement does not include a description of the crime or how the crime occurred.  That information should be included in your witness statement to the police.
