Update company information on file to maintain care and control of your licence and to avoid application processing delays.

Update Signing Authority

Submit a new signing authority each time there is an addition or removal of people from the signing authority.  Make sure to include specimen signatures for each authorized person.

Change of Address

Submit a Change of Address form to change your business mailing address.

Only the signature of the principal owner (for example, the sole proprietor, all partners in a partnership, or director of a corporation) will be accepted. The signature of a delegated signatory will not be accepted on this form.
