CFD Caregiver Screening and Self-Report Audit Tool (SRAT) Questionnaires
Once a contracted agency has begun working with the Ministry of Child and Family Development (CFD) they will need to engage in two separate screening types: Caregiver Screening and the Self-Report Audit Tool (SRAT).
Caregivers and care providers can be approved to provide care for children and youth residing in contracted care settings after they successfully complete the screening and assessment process.

The Self-Report Audit Tool (SRAT) is an online questionnaire that every contracted care provider must complete and submit to the ministry for everyone they hire or sub-contract as a caregiver or care provider for a child or youth residing in contracted care settings. The SRAT questionnaire is based on the standards in the Screening and Assessment of Caregivers and Care Providers in Contracted Care Settings Policy (PDF, 227KB).
