Submission forms
Last updated on May 5, 2026All diagnostic testing and services must include a completed a submission form. Only fully completed official submission forms will be accepted by the BC Animal Health Centre.

| Fish submission form (PDF, 900KB) | Owned and free-living fish species. Includes both Pacific ocean watershed and fish from outside the Pacific ocean watershed. For fish originating outside the pacific ocean watershed, please see the following notification to clients (PDF, 254KB). | May 2026 |
| General ruminant and camelid submission form (PDF, 1.2MB) | Owned small ruminant and camelid species (no bovine). Permanent captive zoo/collection wild ruminant and camelid species. | May 2026 |
| Porcine submission form (PDF, 677KB) | Owned porcine species. Permanent captive zoo/collection wild porcine species. | May 2026 |
| Release of animal remains for cremation (PDF, 60KB) | If you wish to have an animal’s remains released from the AHC to a private crematorium for cremation, include a completed form in your submission. | July 2021 |
| Wildlife submission form (PDF, 650KB) | Free-living (wild) animal species, or wild species in rehabilitation/captivity with intention to release into the wild. | May 2026 |
