You may need to submit additional forms or supporting documents when applying for services.

Contaminated Sites Services Application (CSSA) 

When it's required: 

Used to request ministry services related to site investigation and remediation.

Required Documentation

Forms

  • Completed CSSA form
  • Site Risk Classification Report (if applicable) 
  • Summary of Site Condition 

Supporting documents

  • Detailed Ecological Risk Assessment Checklist (if applicable) 
  • Supporting technical reports (e.g., Phase I/II ESA, remediation plans) 
Additional Notes 
  • Ensure all documents are current and signed by an Approved Professional where required.

  • Completed Contaminated Sites Services Application (CSSA) 
  • Summary of Site Condition
  • Site Risk Classification Report (if applicable)

Supporting documents

  • Remediation plan
  • PSI (if required by SDM)
  • DSI (if required by SDM)
  • Exposure Pathway Questionnaire (if applicable)
  • Copies of any approvals previously issued by the ministry including those provided under Protocols (if required)
  • Current printout of the land title record (or records) and Land Title Office legal plan(s) for the site
  • Current Selection List from an area-based Site Registry search (0.5 km radius) and a Site Registry Detail Report for the site
  • Written consent from the owner to obtain the certification document and to fulfil conditions as required for applicants who are not owners of the property receiving the document
  • Records of communications (if there are affected property owners due to contaminant migration)
  • Engineering designs of works installed to remediate the site, stamped by the engineer
  • Financial security documentation (if applicable)
  • A summary of other legislation, regulations and bylaws that may be applicable to remediate the contaminated site

Notification of Independent Remediation 

When it's required:

For notifying the ministry of voluntary remediation activities.

  • Site Risk Classification Report (if applicable)
  • Site plan
 

Notification of Likely or Actual Migration 

When it's required:

Required when contamination is suspected or confirmed to have migrated off-site

  • Site maps showing migration pathways 
  • Communication records with neighbouring landowners

Site Disclosure Statement (SDS) 

When it's required:

Required when a property is subject to Schedule 2 activities

Required documentation

  • Completed SDS form

  • Historical site use documentation 
  • Environmental assessment reports (if available) 
  • Site maps and aerial photographs 

Additional Notes 

  • Ensure the SDS is signed by the appropriate party (e.g., owner, operator). 

 

Site Risk Classification Report 

When it's required:

Used to assess the risk level of a contaminated site

Required Documentation

  • Completed Site Risk Classification Report
  • Exposure Pathway Questionnaire 

Supporting documents

  • Supporting environmental data 
  • Risk assessment methodology 

Additional Notes 

A Site Risk Classification Report must be prepared by a Qualified Professional.

 

Summary of Site Condition 

When it's required:

Provides a summary of the environmental condition of a site. 

Required Documentation

Forms

  • Completed Summary of Site Condition form 

Supporting documents

  • Recommendation from Approved Professional 
  • Supporting technical documentation (e.g., ESA reports, remediation summaries) 

Additional Notes

  • Include any relevant correspondence with the ministry. 

 

Certificate of Compliance

Required documentation:

Numerical (Not risk-managed)

Forms

  • Contaminated Sites Services Application form
  • Summary of Site Condition (signed by an approved professional)
  • Site Risk Classification Report (if required)

Supporting documents

  • Confirmation of remediation
  • Preliminary Site Investigation (if required)
  • Detailed Site Investigation (if required)
  • Exposure Pathway Questionnaire (if the site is high risk)
  • Copies of previous approvals (if applicable)
  • Current printout of the land title record(s)
  • Current Selection List from an area-based Site Registry search (0.5 km radius) and a Site Registry Detail Report for the site
  • Written consent from the owner to obtain the certification document and fulfil conditions
  • Draft cover letter (if sending via CSAP)
  • Draft Certificate of Compliance (if sending via CSAP)
  • The type of arm’s length review required for the application as per Section 7.3 of the Summary of Site Condition (if sending via CSAP)
  • A summary of deviations from the Contaminated Sites Approved Professionals (CSAP) Society Approved Professional Practice Guidelines (if sending via CSAP)
  • Records of communications
  • Engineering design of any works that were installed to remediate the site, stamped with the seal of a professional engineer
  • Financial security documentation (if applicable)
  • A summary of other legislation, regulations and bylaws needed to remediate the site (if applicable)
