Privacy Collection Notice
The information requested in these forms is collected by the Liquor and Cannabis Regulation Branch under Sections 26 (a) and (c) of the Freedom of Information and Protection of Privacy Act for the purpose of cannabis licensing, compliance and enforcement matters in accordance with the Cannabis Control and Licensing Act. You must submit documentation for fit and proper checks if you are:
- A sole proprietor applying for a cannabis licence
- Identified by the LCRB as an associate of a cannabis licence applicant, or a person with a connection to an associate of an applicant
- For example, a director, shareholder, partner, key personnel, etc.
How to submit your information
- Complete the Cannabis Screening Form [PDF, 224KB].
- Provide the supporting documentation depending on the answer to questions 1 - 4. This may include:
- A Police Information Check (within the last 6 months) from your local RCMP or police detachment OR a Canadian Criminal Record and Judicial Matters check from the Commissionaires
- A statutory declaration signed by a lawyer, notary public, or Commissioner for taking affidavits regarding previous charges and convictions.
- A copy of your driver’s abstract for the past 5 years.
- Upload the completed forms and supporting documents to the liquor and cannabis licensing portal as part of the cannabis licence application.
