NRS Issue Log and Issue Note
The Issues Log records the history and provides an audit trail of all issues raised during the project and summarizes who raised each issue, a description of the issue, who is/was responsible for its resolution and its latest/final status.
Refer to the Issues Plan when planning the approach to issues management.
Standards and Guidelines
The standard is to use the template provided to guide the content of the Issue Log and Issue Notes.
An Issue Log is mandatory for all project levels and categories.
An Issue Note template is available for Project Manager (PM) and Business Portfolio Manager (BPM) use, as required.
