Converting to Individual Benefits Plans
The conversion policy lets you convert to an individual extended health, dental and life insurance plan when your group coverage ends.
You can convert to some or all of the plans below. The carrier must receive the application within 60 days of the end of the month in which the group coverage ends. You cannot retroactively convert your plan. Once you miss the deadline, you are no longer eligible.
Individual Extended Heath & Dental Plans
Once employment ends, you may purchase individual coverage. Please note that an individual plan will be different than a group plan.
Individual Group Life Insurance Plans
If your employment ends or you reach age 65 (and are no longer eligible for group life insurance), you may convert your coverage to an individual policy, limited in both amount and plan, without a medical examination. Or, you may take a medical examination (paid for by the carrier) and choose any insurance plan offered by the company. If you do not meet the medical requirements, you will still have the opportunity to convert your coverage to an individual policy, limited in both amount and plan.
If you do not take a medical examination, you may be eligible for any amount of coverage up to the amount of coverage (maximum $200,000) in force at the time your group coverage ends. The premium for the individual policy will depend on age and on the type of policy you select. It is not the same rate as paid while covered under the group plan.
Converting Spouse’s Optional Life Insurance
If your spouse is under age 65, you may also convert your spouse’s optional life insurance to an individual plan at the same time as you are converting your own coverage.
If your spouse is older than you when you turn 65, they are ineligible for conversion to an individual plan.
