A doctor’s certificate is a form completed by a qualified medical practitioner regarding your medical condition while on short term sick leave. This kind of information will help your doctor and supervisor communicate about your work limitations, restrictions and return-to-work prognosis. It can also provide diagnostic information to occupational health and rehabilitation specialists so they can make recommendations regarding a successful return to work. Your supervisor will provide a blank doctor’s certificate when needed. Have your doctor complete the form (there may be a cost for this service).
