How to Report a Medical Assistance in Dying (MAiD) Death

The Coroners Act requires physicians and nurse practitioners to report the death of anyone who has received medical assistance in dying.

Reporting is accomplished by submitting the BC Coroners Service Report of MAiD Death form (PDF), along with the Medical Certification of Death and all relevant provincial forms from the Ministry of Health, to the BC Coroners Service via facsimile to: 250 356-0445.
