The following forms are available for physicians wishing to enroll in the Alternative Payments Program (APP), the Primary Health Care Blended Funding Model, or any of the Rural Practice Programs.

Note: The Rural Locum Programs and their related forms are managed by Locums for Rural BC. 

Alternative Payments Program

This application form is for physicians who are compensated through fee-for-service (FFS) and seeking to move to compensation through an Alternative Payment Arrangement (APA) to deliver the same clinical services.

Rural Practice Programs

Health authorities complete this form to apply for recruitment incentive payments for physicians recruited to eligible rural communities.
